Jan Steen, a master of Dutch Golden Age genre painting, frequently turned his attention to the absurdities of daily life. In this work, he depicts a travelling charlatan performing a tooth extraction on a patient. The scene is set upon a makeshift wooden stage, a common sight at village fairs during the seventeenth century. The doctor, dressed in a long black coat and hat, holds a tooth aloft as if to prove his success, while the patient grimaces in pain.

Steen populates the composition with a variety of figures that reflect the social hierarchy of the time. To the right, a woman assists the doctor, while a man in a wheelbarrow appears to be recovering from his own procedure. The crowd gathered around the stage includes children, peasants, and a rider on a donkey, all observing the spectacle with varying degrees of curiosity and scepticism. A small monkey sits atop a post, a traditional symbol of folly and deceit in Dutch art, which reinforces the satirical nature of the scene.

The painting is characterised by its narrative density and the artist's ability to capture human expression. Steen uses a warm, earthy palette to ground the scene in a realistic setting, while the lighting directs the viewer towards the central action on the stage. The inclusion of everyday objects, such as the basket of eggs and the medical paraphernalia on the table, adds to the authenticity of the depiction. This work is typical of Steen's approach, which often combined humour with a moralising undertone, inviting the viewer to observe the gullibility of the crowd and the manipulative tactics of the itinerant healer. The composition is balanced, with the large tree providing a canopy that unites the disparate elements of the fairground scene.
