The Crucifixion is the central panel of the first view of the Isenheim Altarpiece, commissioned for the monastery of St. Anthony in Isenheim. Matthias Grünewald employs a stark, emotive realism that departs from the idealised forms common in the Italian Renaissance. The figure of Christ is depicted with a raw, physical suffering, his body marked by lacerations and his fingers contorted in agony. The greenish, pallid skin tone contrasts with the dark, oppressive background, which suggests a desolate landscape under a night sky.

To the left, the Virgin Mary is shown in a state of collapse, supported by John the Evangelist. Mary Magdalene kneels in prayer, her posture conveying a sense of desperate grief. On the right, John the Baptist points toward the crucified figure, accompanied by the sacrificial lamb. The inclusion of John the Baptist, who died before the crucifixion, is a theological device intended to represent the transition from the Old to the New Testament. The inscription 'ILLUM OPORTET CRESCERE ME AUTEM MINUI' (He must increase, but I must decrease) is visible near his hand.

Grünewald uses light and shadow to create a sense of volume and weight. The drapery of the figures is rendered with heavy, angular folds that add to the overall tension of the scene. The composition is balanced but asymmetrical, drawing the eye toward the central cross. This work is noted for its uncompromising depiction of human pain and its ability to communicate theological concepts through visceral visual language. The original altarpiece was designed to provide comfort to the patients of the hospital at the monastery, who suffered from ergotism and other skin diseases, by mirroring their own physical condition in the image of the suffering Christ.
