The scene captures the miraculous vision of Pope Gregory the Great, who, while celebrating mass, witnessed the appearance of Christ as the Man of Sorrows upon the altar. The composition is organised with a direct, graphic clarity typical of fifteenth-century block prints produced in the Low Countries.

In the foreground, Pope Gregory kneels in prayer, his vestments rendered with simple, bold lines. Behind him, an attendant holds a liturgical vessel. The altar area features the central figure of Christ, displaying his wounds, surrounded by the instruments of the Passion, known as the Arma Christi. These symbols, including the cross, the sponge, and the spear, serve as visual reminders of the narrative of the Crucifixion. The surrounding figures, likely representing members of the clergy or congregation, are depicted with stylised features, reflecting the aesthetic conventions of the period.

The use of hand-applied colour, primarily red, green, and ochre, provides a sense of warmth to the stark black outlines of the woodcut.
